Before filling out this form, please check the following:

• Both usernames and passwords are case sensitive, check that you're entering both all in lower case
• Letters and numbers are somtimes hard to tell apart, particularly 0/O and 1/l To help this, automatically-assigned passwords will never have the digits 0 or 1 in them, try your password again with letters instead.
• If you have changed your password recently, you may have to keep using your old one until all the web servers catch up with the change.

If none of those help, or you genuinely have forgotten your password, then fill out the details below and we will attempt to assign you a new password. For security reasons this form will first send a verification code to the address you enter to verify it belongs to you before looking up your account information. We can only send new passwords to an address already in the database.
